The project will contribute to the continued development of the evidence base for digital social care records and falls prevention, evaluate the impact and share the benefits across our integrated Care System footprint. NHS Somerset have made Thick & Easy™ Clear FIRST LINE ON THE PRESCRIBING FORMULARY. This is to support a system wide approach to the patient safety issues in dysphagia patients and the IDDSI Framework.

The Somerset Speech and Language team have asked that:
- Any patient discharged on Thick & Easy™ Clear in an acute setting should remain on it.
- Any new referral patient should be prescribed Thick & Easy™ Clear.
- Where appropriate, all care and nursing homes should use one thickener (Thick & Easy™ Clear). This ensures patient wellbeing as multiple brands may cause dosage and safety issues.
- Patients who have an historic use of a starch thickener for GI tolerance issues, e.g Thick & Easy™ Original, should remain on it.

For general advice regarding laxatives;
- Laxatives and thickeners tend to oppose each other due to their different modes of action and this can make thickening difficult.
- Gum based thickeners (e.g. Thick & Easy™ Clear) tend to be more successful than starch based thickeners when thickening osmotic laxatives.
- The laxative will significantly increase the time required to thicken, therefore it is important to ensure the solution has finished thickening before it is given to the patient.
- Try making up the laxative as directed and then add this solution to a dry cup containing the thickener powder. Alternatively place both powders together (laxative and thickener) into a dry cup and then add the water.
- Using hot water is effective at reducing the thickening time required.

Oral health is an essential part of everyone’s overall health.

The Care Quality Commission expects registered managers to take account of nationally recognised guidance, including guidance from NICE.
Evidence about how you support residents to maintain good oral health will help you demonstrate that your service is both effective and responsive.
NICE’s guideline on oral health for adults in care homes, including the baseline assessment tool, can be used as part of your preparation for inspection and to support requests for help to other services.

Action/Request:
Respond to the regional survey findings and address the recommendations – recruit members for a co-production task and finish session.
Aim:
Develop an oral health improvement offer available across all care homes, nursing and residential, nurses and LD. Launch a blended offer (face to face, virtual, cascade, champion) delivered by Somerset Oral Health Improvement Service in collaboration with Health Education England.

It’s great to do some physical activity, however, even better if, as well as this, people break up their sedentary/sitting time regularly as well.
