End of Life
Somerset End of Life Care and Bereavement Support: A new resource website for Somerset
We are delighted to bring a new resource to Somerset that brings together all the resources, information and education needed to help care and support the people of Somerset living with life-limiting conditions. This website has been a collaborative effort across organisations countywide and we hope you find it informative and useful.
https://somerset.eolcare.uk/
The website has information linked/specific to:
- Place of work
- Topic of interest within palliative and end of life care
- County-wide education
- A section for the public to inform them more about services and pathways in Somerset
If you would like a short 10 minute presentation for your staff/teams on the website and all its sections and functions, please don’t hesitate to get in touch, we’d be only too happy to help via Teams or face to face. Email EOLCEducation@SomersetFT.nhs.uk for more information.

Staff Wellbeing
There are a number of resources available to support staff wellbeing.
Training:
REACTMH® Active Listening Skills conversation training enables Managers/Supervisors to identify and engage in meaningful wellbeing conversations, using “active listening” techniques, to identify and support colleagues struggling with pressures that are impacting their daily mental health.
We have training dates for both REACT90 MH training and REACT MH Train the Trainer for April and May 2022.
REACT90 MH Training Dates April – June 2022 and sign up details
REACTMH Train the Trainer session, a two and a half hour course to provide managers with the knowledge and skills to train other managers in the REACTMH techniques. REACT TTT Training Dates April – May 2022 and sign up details.
Going forward we will be looking to ensure REACTMH Active Listening Skills training becomes embedded within Somerset, and we would love to hear from any of you who have done the training and now would like to become a trainer within your organisation!

This is a resource to support food fortification for those caring for people who have been identified as at risk of malnutrition, or who are malnourished. It is likely to be particularly useful for those working in, or supporting care homes for older adults, but could also be used to support people living in their own homes.

Personalised Care
The South West Integrated Personalised Care Team would like to offer online training and other resources to care providers. Please see below for links to the PCI Institute and the South West Integrated Personalised Care You Tube Channel to share with your colleagues.
1. Register with the Personalised Care Institute and in ‘Training’ they will see e-Learning opportunities. A great introduction to personalised care, they can register for any one or all the short e-learning modules:
- Core Skills
- Shared Decision Making
- Personalised Care and Support Planning (PCSP)
Each module can be carried out in a single sitting or can be completed in more than one sitting when time allows for them to complete it. Completion and evaluation of these modules gives Continuous Professional Development (CPD) points for those who require them, and a certificate is produced for their portfolios.
2. Have a look at the South West Integrated Personalised Care You Tube channel – brilliant short films that say so much, you could start with The Girl Who Climbs and then discover more. We are happy to receive feedback about them. Tell us what you think and of these films help share what personalised care can be and what it means to you. Email us at swipc@nhs.net
3. The poster below can be printed or shared digitally by email to explain the benefits of Personalised Care and the shift in relationships between those who have caring roles with people as well as health and care professionals. The shifts in the type of conversations that are had with people can really enable and empower them to take control of their own care needs and life in general.
