Somerset’s Early Help and Prevention Model

Somerset Council’s Early Help and Prevention strategy is a central component of the Adult Social Care Strategy, focused on supporting people to remain independent, resilient and well for as long as possible, while reducing avoidable demand on statutory and acute services. It is delivered through a whole‑system, place‑based approach that integrates Adult Social Care, health, housing and the Voluntary, Community, Faith and Social Enterprise (VCFSE) sector, with a strong emphasis on community‑rooted support, timely access to information and advice, and strengths‑based interventions.

Somerset uses the Joint Strategic Needs Assessment (JSNA) as a key evidence base to inform the design, targeting and delivery of its Early Help and Prevention strategy.

Delivery has been enabled through our Connect Somerset approach since 2018, that includes social prescribing, community hubs, Village Agents, micro‑providers and targeted commissioned services.

The Council monitors impact and effectiveness through strategic governance, commissioning and contract oversight, shared outcomes frameworks and performance intelligence, using both qualitative and quantitative data to evidence improved wellbeing, increased independence and value for money.
