Digitisation in Social Care

The collaborative three year project between Somerset Council and NHS Somerset reaches its second year. Grand funding up to 50% is available for the first year of licenses and implementation costs for adult social care providers – homecare and domiciliary care providers.

DSCR is saving time…

A digital social care record or DSCR keeps the information you would normally create on paper, on an electronic form, held securely and easily accessible for staff and managers in a timely manner.

DSCR can save 164 hours per year per resident. We have actively saved 255,840 hours this year. With data entry times going from 45 minutes per resident per day to 18 minutes per resident per day. It has positively impacted 1560 lives across 30 care homes.

Find out more about Somerset’s Dementia Wellbeing Service during Dementia Action Week 15 -21 May


This year, during Dementia Action Week (15-21 May), the Alzheimer’s Society will be focussing on increasing diagnosis rates for people with dementia so people living with dementia can get the support they need.

In Somerset, our Somerset Dementia Wellbeing Service; a partnership led by the voluntary sector in partnership with NHS Somerset and Somerset Council, is being rolled out to provide support to thoseliving with dementia and their carers. This includes :-
  • A roadshow with events around the county to showcase options available to support people with dementia and their carers, and to give people an opportunity to experience a free mobile dementia simulator experience on the Somerset Dementia Bus
  • Doubling the number of Dementia Support Workers (provided by the Alzheimer’s Society and funded by NHS Somerset), in Somerset so that every person diagnosed with dementia can be connected with support in the community and guided through their next steps
  • A localised version of the Dementia Connect phoneline, 01458 251541, to connect people directly with the Dementia Support Workers team who can offer information and practical guidance to help people understand the condition, cope with day-to-day challenges and prepare for the future.
  • A new website to connect people with local dementia services and resources.

During Dementia Action Week we will be posting content on our website and our social media channels to encourage people living with dementia and their carers to share their stories. We will also have NHS Somerset’s clinical lead on dementia GP Peter Bagshaw and the Alzheimer’s Society providing advice on support available through your GPs and the Alzheimer’s Society.

How to get a diagnosis

Following a sustained drop in dementia diagnosis rates for the first time ever, the Alzheimer’s Society undertook research to understand the key barriers and benefits to getting a diagnosis.

As well as the misconception around memory loss just being part of getting old, it’s research found being in denial, and referral times to specialists, are big barriers for those experiencing symptoms to seek a diagnosis.

Getting a diagnosis can be daunting, but we believe it’s better to know. If you think you or someoneyou care for may have dementia, please contact your GP to see if it’s appropriate for you/them to bereferred into a Memory Clinic.

Somerset Urgent Community Response

Helping patients access urgent care in their homes (including care homes) within two hours of a referral

Given a choice, many people would prefer to be cared for at home rather than having to go to hospital. If you, or someone you care for, is having a crisis in their health, Somerset Urgent Community Response can help.

The Somerset Urgent Community Response team will send one of their highly qualified health professionals out within two hours of receiving a referral. They are available for anyone over 18 and are open 365 days a year from 8 a.m. – 8 p.m.

What is urgent care?
You can call the team if someone has suddenly deteriorated, for example, when they have become suddenly become less mobile, have had a fall, their informal carer or end of life arrangements have broken down, or they are experiencing confusion or delirium.

In safe hands
The NHS team at Somerset Urgent Community Response brings together a wide range of skills including advance clinical practitioners, district nurses, physiotherapists and pharmacy technicians.

Their teams have lifting equipment so they can help a patient who has fallen to get back on their feet.

Importantly, they don’t just treat the symptoms, but will carry out an assessment to understand why the patient has become unwell. Where necessary, they can refer them onto other health and social care services for help to try and prevent a crisis in their healthcare happening again.

Social Work Sessions podcast

We are thrilled to announce the first of a series of podcasts by Somerset Council's Principal Social Worker (Adults), Carolyn Smith.

This podcast series makes space for conversations about contemporary issues in social work with adults to support learning and development.
In the first of our series, Carolyn chats with Professor Michael Preston-Shoot about self-neglect in social work. Michael is Professor emiritus* of social work at the University of Bedfordshire. He is also the Independent Chair of Somerset Safeguarding Adults Board
